Adenomyosis is a form of endometriosis. The main difference of this form is that the endometrium, breaking through the protective layer of the uterus, grows into its muscular layers. At the same time, as a protective reaction, the uterus forms muscle seals around the implanted endometrium to prevent further growth.

Let's take a closer look at the structure of the uterus to understand what adenomyosis is. The uterus itself consists of several layers - the myometrium, ie the muscular layer of the uterus and the endometrium, the mucous layer that covers the inside of the uterus. It is the rejection of the endometrium at the end of each cycle, which is followed by bleeding. The endometrium itself is a bilayer, ie. consists of a basal layer - basal, which serves as a basis for the growth of a new endometrium and functional, this layer is exfoliated monthly. The functional layer consists of a single layer of cylindrical cells, including cells that produce mucus-glandular cells. It is a functional layer in the pathology that grows into the muscular body of the uterus "breaking through" the basal layer.

Causes of adenomyosis

There is no single theory today that fully answers the question of why adenomyosis occurs. Each of the advanced theories can partially explain its appearance but does not provide complete answers to all questions. Here are the most common theories about the causes of adenomyosis:

  1. Implantation - this theory explains the occurrence of endometriosis due to a sharp increase in blood in the pelvic organs and peritoneum, but if this can explain endometriosis, then the occurrence of adenomyosis is difficult to explain.
  2. Coelomic epithelial metaplasia - this theory explains the onset of adenomyosis by the fact that not all embryonic tissues undergo regression before menstruation, it is inserted into the myometrium.
  3. Induction - this theory largely coincides with the previous one and indicates the possibility of adenomyotic foci under the influence of negative external factors.

The second or third theory may partly explain the occurrence of adenomyosis in young patients, but evidence for these theories has not yet been provided. In addition to the theories described above, there are others - hormonal (prostaglandin deficiency), genetic and others, but they still remain theories.

In addition, there are a number of factors that can increase the risk of adenomyosis. These include:

  • Infections of the urogenital system.
  • Abortion and scraping.
  • Intrauterine intervention.
  • Injuries, including generic.
  • Disturbed hormonal background.
  • Genetic predisposition.

Types of adenomyosis

Uterine body adenomyosis is divided into nodular, focal, and diffuse. There are four stages in the development of this pathology:

I - the endometrium grows to the myometrium,

II - the endometrium penetrates into the middle of the muscular layer of the uterus,

III - the endometrium reaches the serous lid,

IV - endometrium penetrates the peritoneum.

The main symptoms of adenomyosis

  1. Menstrual pain or algomenorrhea is the most common symptom. It is especially worth paying attention to if it occurs in adolescents. The pain occurs due to the accumulation of fluid in the tissues and the local inflammatory process, as well as due to the accumulation of blood.
  2. Menstrual irregularities are another symptom of adenomyosis. It most often manifests itself in the form of bleeding. There may be brown discharge before and after menstruation, while menstruation itself may last longer and with more bleeding than usual.
  3. Infertility - as a rule, is not so much a consequence of adenomyosis, as a combination of several factors. When the process spreads to the entire uterine cavity, active adhesive processes are possible, and if adenomyosis is accompanied by other diseases, such as endometriosis, fibroids, and others, then the chance of getting pregnant is drastically reduced. Adenomyosis itself can not become a barrier to pregnancy with proper treatment and prevention.
  4. Disorder or miscarriage - is also the result of a combination of several pathological conditions.

Diagnosis of adenomyosis

  1. Depending on the size and structure of the uterus, the size of the uterus, depending on the stage and development of adenomyosis, may increase or decrease at 5-8 weeks of pregnancy or normally. The body of the uterus is often compacted and in some cases a lump. Palpation before menstruation or the patient may feel painful. Possible and pain and induration of the cervix. The uterus itself may lose its normal mobility.
  2. When ultrasound tests diagnose adenomyosis it is much easier, especially if you are having a vaginal examination. With this type of diagnosis, the accuracy of the diagnosis will be 90%. It is better to perform an ultrasound on the cycle of 22-25 days.

An ultrasound scan may show such signs of adenomyosis:

  • increased echogenicity of the myometrium
  • increased uterine size
  • the presence of small inclusions, 0.02 to 0.6 mm in diameter. At the same time, an experienced doctor will be able to distinguish an adenoma node from a fibroid due to the lack of a capsule in it and in clear forms.
  1. MRI is used less frequently, but using this method, the condition of the myometrium and the presence of adenomyotic foci in them can be assessed.
  2. Hysteroscopy. The most effective and informative diagnostic method. It allows you to accurately determine the condition of the myometrium and more accurately determine the stage of the disease and prescribe appropriate treatment. The disadvantage of this method is that the patient must be placed under anesthesia. The hysteroscopic classification of adenomyosis is as follows:
  • In the first stage, small foci are visible, but the walls of the uterus do not change.
  • In the second stage, adenomyotic "movements" are observed, the uterus is poorly stretched and has uneven walls.
  • In the third stage, different sizes of "protrusions" appear in the uterine cavity, which does not have clear contours. There may be adenomyotic "movements" of the eruptions, but not necessarily.
